Membership to the ACFP is managed by the College of Family Physicians of Canada—both national and provincial memberships are required. There are several membership categories available to you.  Below is an excerpt taken from the CFPC site.

Active

Download application to join as an Active member.

Licensed Physician in good standing, engaged in the practice of family medicine

Annually at the member’s birth month.

250 credits (total of MAINPRO-M1, M2 and/or C) must be submitted in each 5 year cycle; at least 125 must be MAINPRO-M1 and/or MAINPRO C

Resident

Physician enrolled in an approved postgraduate training program Annually each July. Terminates upon ceasing to be enrolled in an approved postgraduate training program. N/A
Senior Licensed Physician in good standing, aged 65 or over who has been a member in good standing for at least the 10 consecutive years immediately prior, engaged in providing medical care to patients or in a medically related field. Annually at the member’s birth month. Same as "Active"
Retired Physician who is not actively engaged in providing medical care to patients or actively involved in a medically related field. Annually at the member’s birth month. N/A
Affiliate Specialist

Download application to join as an Affiliate Specialist
A Licensed Physician in good standing, who holds Certification from the Royal College of Physicians and Surgeons of Canada or other recognized certifying body outside the College and whose primary practice is in a specialty other than family medicine. Annually at the member’s birth month. Same as “Active.” Credits from equivalent programs (e.g. Royal College) may be submitted in lieu of MAINPRO.
Student

Fill out an application for a Free Student membership.
Individual enrolled in a Canadian university faculty of medicine undergraduate program leading to the MD degree. Annually each July. Terminates upon ceasing to be enrolled in an approved undergraduate training program. N/A
Associate



Download application to join as an Associate.
Members of the public who are serving or have served as Directors of the National Board of the College or allied health professionals or other non-physicians who work in collaboration with family physicians or academic departments of family medicine.
Annually at the member's enrolment month. N/A
